Job Description
Summary:
Plans, directs and integrates quality, resident safety, resident experience of care and compliance for HighPointe on Michigan and DeGraff Rehabilitation and Skilled Nursing Facility. Interprets and implements quality assurance, performance improvement and safety standards to ensure quality care to residents; oversees the audit of work activities to verify conformance of activities to regulatory and quality standards; maintains ongoing tracking and reports of findings; integrates quality and safety with risk management activities. Ensures that the facility/entity activity is consistent with the system's overall Mission, Vision and Strategic Plan. Assures compliance with Federal, State, Local and Accrediting bodies for NYS. Supervises Managers LTC MDS and Nursing Administration at both sites.
Education And Credentials
Responsibilities:
Bachelor's degree in Physical Therapy required. Bachelor's degree in Nursing preferred
Experience
Other information: 5 years of experience in Working in a New York State Skilled Nursing facility with at least two years in a nursing/clinical leadership role required. 2 years of experience in a healthcare Quality Assurance Performance Improvement role preferred.
