Job Description
Responsible for the overall accountability for the daily coordination and operations of Care Coordinator members provided by Oishei Healthy Kids Health Home. Supervise a team of 5-8 Care Managers. Provide services to children/adolescents up to the age of 21. Ensures that the Care Coordinators use a holistic approach to care and that goals for all dimensions of health; psychosocial, behavioral, emotional and medical are met. Collaborate with Social Work, all Care Coordinators, Medical Staff, Nursing, Ancillary services to ensure that children/adolescents care is coordinated, with all accompanying documentation and referrals, appropriate community referrals, and completed family/children/adolescents education and linkages. Supervises staff, oversees daily program activities and ensures ongoing staff development. Provide oversight, reviews and updates procedures to assure quality indicators are met and facilitates change when necessary.
Education And Credentials
BA - Bachelor of Arts Social Work, Human Services or related health care field required.
Experience
3 years of experience in providing care to persons with mental illness, chemical dependency, developmental disabilities and/or a chronic medical conditions required. 1 year of experience in a supervisory capacity with Caseworkers, Coordinator Care, Managers or similar Care Management positions preferred. 1 year of experience in Health Home Setting preferred.
