Key Responsibilities :

Empower Independence : Support individuals in everyday activities at home and in the community, helping them build skills and confidence.

Facilitate Essential Activities : Assist with grocery shopping, doctor's appointments, and other outings, providing transportation and companionship.

Personalized Goal Achievement : Collaborate with individuals to implement support strategies that align with their interests and personal goals, including physical activities and community engagement.

Navigate Systems Together : Aid individuals in completing housing applications and accessing government assistance programs.

Support Financial Management : Help with monthly budgeting and effective household management for daily necessities.

Document Progress : Complete and submit accurate daily progress notes to track each individual's growth and development.

What We're Looking For :

Qualifications : A Bachelor's degree OR a High School Diploma (or equivalent) with at least 2 years of relevant community-based experience.

Driver's License : A valid driver's license and active car insurance are required, with a willingness to drive individuals in your personal vehicle.

Availability : Must be available to work at least 4 hours per day between 8 AM and 5 PM, with flexible hours / days.

Skills : Excellent communication, interpersonal, and organizational skills are essential for success in this role.
