The Community Resource Manager will manage an evolving partnership between families, network partners and community resources with a focus on cultivating relationships with direct service providers and families within our Intact program. The Community Resource Manager will work closely with Kaleidoscope and other THRIVE network partner staff in identifying, coordinating and delivery of services and opportunities in accordance with the Agency's mission and values and core principles.
Position-Specific
- Provide coordination of Thrive project activities such as scheduling Thrive Team meetings, creating agendas and maintaining minutes on agency minute form.
- Participates in weekly supervision, team meetings and department meetings.
- Actively seeks links to current and new community resources appropriate for child, youth and family needs by working with community partners by fostering relationships and collaboration and promoting equitable and prioritized services for families.
- Identify and address improvements to working together to support community participants in engaging in services and supports.
- Attend monthly service area meetings (LAN67) and other related committees and coalitions including, but not limited to Austin coming together, Garfield Park Rites to Wellness, West side minister's coalition.
- Identify ways that flex funds can support families and coordinate with assigned specialist to secure funds.
- Maintain directory and contact information for each agency and service and update contact changes and programs monthly or sooner if needed. Share with the Intact team monthly and at all meetings.
- Complete monthly service notes for aftercare Thrive families.
- Develop monthly community activities with partner agencies based on stakeholder feedback and attend / support events to help facilitate family engagement and success, for example Monthly Thursday eve community dinners.
- Schedules regular meetings with supervisors and specialists to solicit information and provide specific referrals for our Thrive families.
- Attend child and family team meetings, as requested.
- Work with Intact Specialists to determine families' specific needs and resources-within and beyond the scope of DCFS.
- Provide ongoing written (paper and text) and online information and materials designed to support internal and external communication efforts about programming, resources and opportunities such as handouts, sign and postings.
- Make recommendations for database needs, data collection and resources for service needs.
- Participate in the continuous quality improvement and data-driven decision-making processes to assess the efficacy of programming and to drive programmatic changes including data reports to any CQI partners.
- Ensure adherence to data collection requirements using the current logic model.
- Enter referral information and dispositions into client database for resource referrals.
Educational Requirements / General Qualifications :
Minimum Degree Required : Master's degree in social work or social service-related degree.
Degree Preferred : Master's degree in social work (MSW).
Licensure : LCSW OR LCPC licensure preferred.
Information Technology Skills Required : Microsoft Office Proficiency
Other Requirements : Must possess or pass CWEL, CANS, CERAP, and other State of Illinois certifications, as required. Excellent interpersonal, oral and written communication and time management skills. Full time use of minimally insured operating automobile and valid driver's license. Must pass a satisfactory Motor Vehicle Report (MVR).
