What are the responsibilities and job description for the Medical Assistant/Care Manager (BH) position at Kalihi Palama Health Center?
Primary Purpose: The incumbent of this position supports and coordinates integrated patient care for patients receiving Behavioral Health Services at Kalihi-Palama Health Center. The care manager will be responsible for the coordination of care for assigned behavioral health patients with the use of assessment, evaluation, interdisciplinary communication, and processing referrals in order to ensure patients receive high quality and effective healthcare. The care manager is responsible for enhancing continuity of care and care integration to ensure patients receive the necessary services and treatment in order to improve healthcare outcomes, avoid complications, increase adherence with treatment and reduce unnecessary utilization of emergency medical/psychiatric services. The incumbent of this position will also serve as a medical assistant for assigned Behavioral Health providers to ensure a coordinated office visit for Behavioral Health patients to include registration, taking vital signs, administration of injectable medications, and recording information in the electronic medical record. |
Essential Duties: Medical Assistant Duties:
o Records vital signs and other measurements accurately o Updates all information related to meaningful use and CARF requirements o Carries out necessary check in clinical procedures appropriately per protocols
o Carries out clinical procedures and order as directed by the provider efficiently and accurately o Administers medication as ordered by the provider o Refers patients as necessary o Complete laboratory orders accurately
o Completes necessary forms o Follows up on no-shows and schedule as necessary o Sterilizes, labels and packs instruments as necessary o Retrieves and returns calls from patients, staff and other agencies in the community o Calls patients for laboratory and/or other reports o Schedules appointments as necessary
Care Manager Duties: Screen patients according to CARF standards using standardized screening tools (PHQ-9, DAST, GAD, Audit-C) Complete chart audits for assigned providers to ensure CARF documentation compliance Coordinate patient care needs with Behavioral Health Provider for referrals and applications for benefits/resources Utilize interventions to engage patients in treatment and reduce non-adherence and no-show rate for Behavioral Health patients Participate in quality improvement projects to improve healthcare outcomes for behavioral health patients Communicate with interdisciplinary team to ensure continuity of care for patients Stock and rotate medications in the Behavioral Health Department. Document the sample psychiatric medication and ensure proper disposal of psychiatric sample medications. Regular and reliable on-site attendance is required. |
Other Duties: Perform other related duties as assigned Provide back-up coverage to the front desk staff in their absence.

Minimum Qualification Requirements: Skills/Knowledge: Ability to add, subtracts, multiply, and divides in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Demonstrated ability to work as part of an integrated team. Education: High School Diploma/GED Current unexpired Medical Assistant Certification One-year certificate from an accredited Medical Assisting program Current BCLS/CPR Experience: At least 3 years of medical and/or behavioral health experience preferred 1-3 years working in a medical office or clinic required with front and back office experience preferred. Experience working with Centricity or other electronic medical record preferred. Previous experience with basic computer systems. Basic medical terminology required.
