MDS Coordinator Job Description
As the MDS Coordinator, you will play a crucial role in ensuring that our residents receive the best possible care. Your responsibilities will include:
- Conducting thorough assessments of residents' healthcare needs.
- Collaborating with healthcare professionals to develop and update care plans.
- Ensuring accurate and timely completion of MDS assessments.
- Maintaining compliance with state and federal regulations.
- Participating in interdisciplinary care conferences.
- Providing education and support to staff members.
Qualifications: To be successful in this role, you should possess the following qualifications:
- Valid RN or LPN license.
- Knowledge of MDS 3.0 and the RAI process. (If no prior knowledge, training available to learn the role.)
- Strong organizational and communication skills.
- Understanding of long-term care regulations.
- Ability to work collaboratively in a team environment.
