What are the responsibilities and job description for the Community Integration Specialist position at Kalos Specialized Services?
Community Outreach Specialists shall provide services to developmentally disabled or special-need consumers in the following areas on an as needed basis based on individual consumer needs:
- Provide skills training in: money management, psycho-social well-being, bus/destination training, social etiquette and social appropriateness, health/personal hygiene, safety, utilization of community resources, special purchases and self advocacy
- Provide any assistance that may pertain to the overall well-being of the client
- Maintain current and accurate documentation on a per-visit basis
- Report incidents and injuries in a prompt manner
- Assist in reporting to referring agencies
- Attend quarterly and annual consumer staffing meetings
- Maintain good rapport and interaction with consumers
- Maintain good rapport with outside agencies and their personnel
- Comply with consumer Individual Program Plans
- Handle consumer emergencies and crisis situations
This position may be subject to occasional on-call status. Employee may be required to answer consumer emergency calls on an as-needed basis.
Job Requirements:
- Reliable vehicle to be used to transport consumers
- Valid California Drivers License.
- Insurance on the vehicle to be used
- An outgoing and energetic personality and a willingness to help those in need
- First-aid/CPR certified or certification thereof within 30 days of hire.
Job Types: Full-time, Part-time
Pay: $16.00 - $18.00 per hour
Expected hours: 20 – 35 per week
