Plans, organizes, develops, administers, implements, coordinates, and evaluates community health improvement strategies. EXAMPLE OF DUTIES
Facilitates the KaRe to Achieve committee, a Kandiyohi-Renville leadership and public health planning team focusing on local public health assessment and planning initiatives. Develops and conducts the Community Health Assessment (CHA) in conjunction with community partners. Develops, implements, and evaluates the Community Health Improvement Plan (CHIP), a long-term, ongoing, systematic effort to address public health problems in the community, with the goal of equitably improving resident health. Project manages the CHIP work plan by addressing goals, objectives, and strategies.Gathers best-practices, benchmarks set by health planning initiatives and other state and local plans and data sources, and community input related to the CHIP.Provides community health planning expertise to the community needs assessment process, health improvement goal setting, program development, identification of public health ordinance and policy action options, and creation of monitoring and evaluation plans.Recommends and supports development of data-informed strategies and programs in alignment with evidence-based practices and the CHIP or other community initiatives. Analyzes gaps, liabilities, opportunities, and strategies in the context of specific population health challenges facing communities.Leads multi-sector community-centered planning efforts to integrate and implement strategies to improve the community’s health and promote health equity throughout diverse populations.Develops, implements, and manages continuous improvement strategies and organizational strategic planning processes. Assures that Kandiyohi-Renville CHB Local Public Health Assessment and Planning requirements and processes are completed, submitted, implemented, and updated per Minnesota statue 145A. Contributes to the development of organizational competencies that strengthen the quality and performance of public health practice.Participates in organizational capacity building and training activities to advance public health competencies.Leads and facilitates meetings with agency staff, as directed, to analyze process and program performance, identify challenges, and use data to drive decision-making, prioritization, consensus action planning, and evaluation.Assists the CHB and the departments with quality improvement opportunities that refine/enhance the diversity, equity, and inclusion of programs and services.Develops public health/projects/program messaging appropriate for targeted audiences for dissemination through CHB and department webpages, social media, and other public-facing presentations.Participates in and supports community-based coalitions that examine health data, set goals, and develop plans to improve health. Assists with the development and implementation of community outreach and engagement activities and events designed to foster relationships and provide feedback to inform program planning and activities. Analyzes and interprets public health data and prepares narratives and reports that identify gaps, errors, and trends and effectively communicates recommendations, solutions or alternative responses to real or potential health challenges facing communities.Works with management to identify new and creative opportunities to leverage shared data to support additional program improvement processes or functions. Maintains and refines reports, dashboards, and processes to continuously monitor data quality and integrity.
