Community Health Planner
Salary: $25.10-$31.38/HR
Hours: 35 HR/WK
Aurora Location: The Community Health Planner promotes and supports population health by leading community health planning initiatives, including the Community Health Assessment and Community Health Improvement Plan. The Community Health Planner engages with a variety of people to assess the health of residents of Kane County and create plans for improving health. Responsibilities include developing and implementing community health assessment and planning cycles in collaboration with Department staff, staff in other Kane County Departments, community stakeholders, elected officials, and community partners.
Qualifications:
• Master’s Degree in public health, public administration, urban planning or other closely related field preferred. Consideration will be given to Bachelor’s degree in public health, nursing or a closely related field and three (3) years of relevant public health experience.
• Three (3) years of public health experience preferred.
• Proficiency in use of advanced computer skills: email, word processing, presentation and publisher software, and internet searches.
• Proficiency in stakeholder/community engagement/management and public speaking.
• Valid Illinois driver’s license, current certificate of vehicle insurance, and access to reliable transportation to all assigned work locations.
