JOB SUMMARY
The Kinship Navigator Case Manager is responsible for providing services and support to grandparents or other relatives raising children in Sedgwick County. The Case Manager is responsible for conducting assessments information and completing an individualized family service plan. The Case Manager will then refer families to community resources that will accentuate strengths as well as address needs. They will work at cultivating relationships with key community agencies in order to easily access services. During and after services have been rendered, they will monitor a client’s follow-through and the effectiveness of the service provided. The Case Manager develops successful collaborations and partnerships with agencies, churches, schools, government agencies and business leaders in the community who provide services and support to families. The Case Manager will work with families to teach personal advocacy and provide written reports as requested.
ESSENTIAL JOB FUNCTIONS
Assessment Activities
- Assess child and family utilizing interview and strengths-based standardized assessments to determine help determine strengths and the areas of improvement to help ensure a strengthening families approach. Complete an individualized family service plan with family.
- Meet with family and child and gather information from family members and collaterals for the development of the service plan.
- Provide ongoing assessment of strengths and needs and identify resources.
Case Management Activities
- Provide services, support and connection to community resources to help the family to maintain the children in their homes.
- Documents and reports all contact with families.
- Assist family in managing crisis situations due to financial, psychological, legal, medical or any other circumstances that present a danger to the family’s well-being.
- Make referrals to community agencies.
EDUCATION AND EXPERIENCE
Required
- High school diploma or equivalent, some college experience
- 1-3 years of work experience in Human Services field
- Good written and oral communication skills
Preferred
- Bachelor’s degree in Human Services field
- Bilingual in Spanish and English
- Experience working with parents
