What are the responsibilities and job description for the Chaplain- Pipeline position at KANSAS CITY HOSPICE INC?
Assess and provide spiritual support to hospice patients and families.
Assists patients / families in maintaining and re-connecting them with their denominational affiliations.
Provides spiritual counseling as indicated in the plan of care and updates specific interventions in patient's plan of care.
Develops and maintains positive, helpful relationships with clergy and religious leaders in the community.
Serves as a staff resource regarding religious beliefs, practices and rituals.
Participates in bereavement program functions to facilitate continuity of spiritual care, when appropriate.
Conducts funeral and memorial services for families served by the agency or assists families in planning and arranging services, when appropriate.
Provides spiritual and emotional support to hospice staff members and clergy working with hospice patients.
Assists in supervision of spiritual care volunteers when assigned.
Documents direct service and ongoing communication with each patient.
Participates in on-call duties to include evenings, nights and / or weekends to ensure a 24 hr coverage of patient care.
Responds to request for presentations to ministerial groups and other community speaking opportunities, when appropriate.
Participates in agency wide quality improvement.
Accepts and performs other related duties and responsibilities as required.
Uses resources efficiently and prudently, i.e.
medical and office supplies, cellular phones, etc.
Schedules or manages his / her time effectively.
Communicates with patients & families, referral sources, insurance case managers, physicians and others with courtesy, responsiveness and cooperation.
Maintains positive work relationships and contributes to a team environment by : demonstrating respectful and appropriate language and nonverbal behavior; actively seeking to resolve customer and co-worker conflict issues, problems and concerns in a timely manner; and demonstrates respect, courtesy and empathy to all individuals; patients, families, co-workers, physicians and volunteers.
