Patient Care Manager Red Team

Position Summary : The Patient Care manager has 24 hour responsibility for the leadership and management of their designated service area; ensuring excellence in service and quality of care.

The Team Manager reports to the Director of Clinical Services.

Team Manager Accountabilities : People Management : Screens applicants, conducts interviews, and makes hiring decisions in accordance with KCHPC guidelines and standards.

Evaluates employee performance to determine developmental needs, establishes goals, and provides opportunities for staff / professional development.

Manage employees in accordance with KCHPC human Resource policies.

Quality Management : Identifies and initiates performance and quality improvement activities that support the mission and goals of KCHPC.

Monitors care delivery to ensure compliance with regulations and optimal outcomes Demonstrates clinical judgment and decision making based on knowledge and skill in caring for patients with life-limiting illness.

Customer Management : Demonstrates knowledge, skills, and attitudes that support the mission of KCHPC Communicates effectively with patients, families, and health care professionals.

Develops and implements action plans to achieve growth and excellence in customer service.

Financial Management : RN (BSN preferred) from an accredited institution or BSW (MSW or Master's Degree in a related field preferred.) Must be licensed in both Kansas and Missouri.

Two years experience in caring for terminal patients and others with life threatening illnesses.

Motivated to work with the dying and bereaved.

Able and willing to work as part of an interdisciplinary team.

Commitment to quality and excellence in service.

Flexible Good Administrative skills and experience in supervision, management and coordination of the work of others.

Strong organization and communication skills and ability to handle multiple priorities.

Licensed driver and reliable transportation and auto insurance.
