This job was posted by : For more information, please see : PURPOSE : The purpose of the Outpatient Therapist position is to provide individual, family, and couples therapy / counseling services to clients in an outpatient setting. The role involves conducting assessments, developing treatment plans, maintaining accurate documentation, and collaborating with internal and external healthcare providers to ensure comprehensive care.OFFICE LOCATION : Hybrid between Troy and Hiawatha OfficesESSENTIAL JOB FUNCTIONS : Conduct intake assessments with appropriate referrals for other services, diagnostic evaluation, and development of initial treatment plans.Master Treatment Plan development, updates, and collaborations with Community Programs staff as appropriate in planning of care.Consult and coordinate with other professionals, intra-agency, and outside agency health care providers for treatment coordination.Maintain documentation of all consultation and coordination activities.Make referrals to other agencies, professionals, or inter-agency programs as needed for quality of client care.Utilize and complete assessments as required, i.e. Daily Living Activities (DLA -20, CAFAS, PECFAS) Assign Level of Care based upon diagnostics and assessments tools as per agency guidelines.Provide individual, family, and couples psychotherapy / counseling.Maintain treatment documentation / records as per agency policy and procedures which meets state and federal requirements. Collaborative documentation, which isan EvidencedBased Practice, is the agency procedure for clinical record keeping.Maintain productivity by consistently meeting billed hour standard as set by Clinical Director.Submit reports i.e., for foster care agencies, courts, attorneys, PCPs schools, Head Start observations, and so on; written in appropriate professional language and in a timely manner.Provide emergency services during office hours (assigned as needed to assist the Crisis Team) and after hours (on a voluntary rotating basis). These may be provided via TeleVideo or face to face as needed. These services include, but are not limited to the following : crisis psychotherapy, consultation with psychiatric medication provider in crisis situation, assessment / referral and coordination for private acute hospitalization which includes coordination with courts, phone consultations, with law enforcement, hospital or medical staff, and concerned significant others who contact the agency for crisis concerns, coordinate needed services to keep clients safe and to stabilize.{=html} 1. Provide expert court testimony as needed. 2. Provide public information presentations and assist with agency table at community events such as Health Fairs. 3. Participate in clinical and all staff meetings in a professional and respectful manner. 4. Maintain clinical license through required continuing education and other requirements. 5. Adhere to the ethical standards of the profession. 6. Pursue and achieve individual professional goals to grow in professional expertise. 7. Provide appropriate psychological testing along with diagnostic interviews according to training and education 8. Psychological assessments may include, but are not limited to, the following : competency to stand trial, court ordered assessments, and law enforcement evaluations 9. Provide comprehensive, professional psychological evaluation written reports in a timely manner to the referral source
