Function:
- Receives, reviews and coordinates patient clinical referral activity.
- Completes admission assessments
- Serve as Charge Nurse on short term rehab unit
- Inputting physician orders in system
- Assists with arrangements and confirmation of all admissions.
- Coordinates the assignment of attending physician.
- Conducts discharge assessments
- Completes discharge packets
- Conducts immunizations and covid testing for new residents
- Promotes optimal communication among program staff members, facility staff, referrals sources, physiciansand patients.
- Adheres to all safety and risk management protocols and programs.
- Participates in direct and indirect patient care activities as warranted.
- Participates in quality improvement activities and QAPI process to increase efficiency through improved systems and processes
- Exhibits courtesy, compassion and respect to patients, families, visitors, physicians, administrators and coworkers.
- Performs other duties or functions as directed.

Requirements:
- High School graduate.
- If RN, must have a current license to practice profession in state.
- Must have a reliable source of transportation.
- Valid driver’s license and proof of insurance.
- Ability to understand, remember and carry out oral and written instructions in English.
- Ability to work in Excel, Word, Power Point, PCC and Nexion dashboard or have the ability to learn.
- Ability to speak clearly and hear well enough to communicate with others.
- Ability to complete assignments while having constant interruptions.
- Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation.
- At least one year of office or LTC clerical experience.
- Knowledge of medical terminology preferred.
- Ability to type.
- Prior experience in admissions preferred.
