About the Role
">This position is responsible for coordinating patient care and developing comprehensive care plans in collaboration with healthcare teams. A Registered Nurse with experience in hospice care will provide skilled nursing services, facilitate communication among team members, and ensure patients receive high-quality care.
Key Responsibilities:
- Develop and implement individualized care plans based on patient needs and preferences.
- Collaborate with healthcare professionals to ensure seamless transitions of care.
- Provide education and support to patients and families on disease management, treatment options, and end-of-life care.
- Maintain accurate records of patient progress, including vital signs, medications, and treatment outcomes.
- Participate in multidisciplinary team meetings to discuss patient care and develop strategies for improving quality of care.
Requirements:
- Diploma or Associate/Bachelor's degree in Nursing; BSN preferred.
- Licensed as a Registered Nurse in New Jersey; licensure in Pennsylvania if practicing in PA.
- At least one year of adult, pediatric, or medical/surgical clinical experience.
- Prior home care or hospice experience is preferred.
