Description
Starting pay $30.00 per hour. M-F 830a-430p

- Triage clinical calls, pages nurses, transfers calls, takes and distributes clinical telephone messages.
- Orders medical supplies and maintains inventory of supplies, as directed by Clinical Director.
- Assumes backup responsibilities for CHHA Scheduler in their absence.
- Assumes backup responsibilities for field LPN.
- Maintains and updates case management sheet weekly.
- Tracks Face to Face visits and communicates with physician and nurse practitioners regarding due date.
- IDT preparation, including making COTI's, scribing during meeting.
- Sends daily fax communication to Medical Director regarding new admits.
- Retrieve information regarding deaths and schedule DME Pickups.
- Assumes backup responsibilities for Referral Services.
- Keeps and tracks accurate DME records
- Provides feedback to referral sources on referrals as indicated.
- Responds to telephone inquiries regarding available services and eligibility for same.
- Utilizes SFAX to send and track all orders.
- Provides coverage at the main office, hospice house, and Pike office as needed.
- Participates in all required in service education programs and meetings.
- Demonstrates a commitment to own work schedule by adhering to attendance standards. Provides proper notification in advance for absence and tardiness.
- Adheres to employment policies in accordance with the Employee Handbook
- Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
- Performs other duties, as assigned.
Position Requirements
Required Skills / Abilities :
Education and Experience :
Required Education : Graduation from an accredited school of Practical Nursing; licensed to practice as a Practical Nurse in the State of New Jersey.
Preferred Education : Some college or business school.
Required Experience : Minimum one year adult or pediatric medical / surgical clinical experience.
Preferred Experience : Prior home care or hospice experience preferred.
