Job Description:
The RN Case Manager Specialist will be responsible for coordinating patient care and services, ensuring that patients receive high-quality, compassionate care in the comfort of their own homes. This role requires strong organizational and communication skills, as well as the ability to work collaboratively with others.
Key Responsibilities:
- Coordinate patient care and services, including scheduling appointments and meetings
- Develop and implement individualized plans of care for each patient
- Collaborate with the interdisciplinary team to ensure seamless care delivery
- Communicate effectively with patients, families, and healthcare providers
- Document all interactions, observations, and interventions in the electronic medical record
Requirements:
- Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred
- Licensed to practice as a Registered Nurse in the State of New Jersey; licensure in Pennsylvania if practicing in PA
- Minimum one-year adult, pediatric, or medical/surgical clinical experience
- Prior home care or hospice experience preferred
