Responsibilities
- Coordinate and assign caregivers to clients based on their needs.
- Conduct client assessments and home visits to evaluate service requirements and develop personalized service plans.
- Monitor the quality and hours of service delivered, ensuring client satisfaction and adjustments as needed.
- Engage with clients to track progress and maintain high levels of satisfaction.
- Work collaboratively with the Field Service Coordinator and Marketing team to understand patient needs and ensure appropriate caregiver placements.
- Manage phone calls and provide administrative support.
- Maintain patient confidentiality and privacy with respect and dignity.
- Be available for on-call scheduling and case management as needed.
- Travel occasionally to open new cases and visit existing clients.
Requirements
- Fluency in English other languages is plus.
- Sales skill, Excellent follow up skills.
- Proven experience in scheduling, case management, or a related field.
- Strong understanding of case management principles.
- Excellent communication, organizational, and time management skills.
- Proactive problem-solving abilities and the capacity to handle multiple tasks.
- Proficiency with Microsoft Office and scheduling platforms.
- Ability to work independently and as part of a team.
- Problem solving skills and ability to multi-task.
- Valid Driver’s License.
Job Type: Full-time
Pay: $17.50 - $19.98 per hour
Expected hours: 40 per week
