POSITION SUMMARY/ROLE: The Patient Care Manager is responsible for managing high risk chronic illness patients to promote effective education, self-management support and timely healthcare delivery to achieve optimal quality and financial outcomes. The Patient Care Manager will work as part of the Patient Care Team to insure effective and quality care is given to the KVHC patient.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
1. Collaborate with physician and center staff in identifying appropriate patients for care management, utilizing established case management criteria.
2. Conducts initial and periodic assessments for case managed population. Prioritizes patients according to intensity, need and required follow-up.
3. Formulate and implement a care management plan that addresses the patient’s identified needs by assessing the patient/family needs, issues, resources and care goals. Establish a care management plan that is mutually agreed upon by the health care team and the patient/family. Evaluates and revises the plan as needed to reflect changing needs, issues and goals.
4. Identify and effectively utilize community resources to meet the needs of patients/families.
5. Promote patient self-management to achieve maximum levels of wellness and independence.
6. Assist the patient to identify and/or develop their support system and encourages the patient to utilize that support.
7. Collaborate with physicians and other healthcare team members, i.e. Patient Assistance, Referral Department, to facilitate care across the healthcare continuum and optimize clinical outcomes.
8. Serve as a liaison to providers, patients and families for coordination of services.
9. Serve as a patient advocate.
10. Monitor patient utilization of emergency rooms and hospitalizations and arrange for follow up with Primary Care Provider.
11. Maintain databases on care managed population.
12. Maintain accurate and timely documentation.
13. Collaborate with Quality Team staff and outside entities providing timely and accurate reporting of clinical quality measures of defined populations.
14. Provides reports on patient care management for the QI/QA committee.
15. Utilize acceptable standards of care and Katahdin Valley Health Center Protocols and Policies as the foundation and framework for chronic illness care management.
16. Participate in the Patient Care Team as outlined in Protocol ADMN 118 Team Approach to Care Protocol
17. Perform other duties, projects as assigned by the Clinical Coordinator.
MINIMUM REQUIREMENTS
1. Graduate of an accredited School of Nursing
2. Current License as a Registered Nurse by the State of Maine
3. Minimum of two years professional experience in related field.
4. Previous work experience in patient care management - preferred
5. Experience in healthcare administration.
6. Excellent interpersonal and problem solving skills
7. Excellent verbal and written communication skills
8. Must possess computer and keyboarding skills with experience in Microsoft Office Products
8. Be self motivated and able to work independently with supervision as necessary.
PERFORMANCE STANDARDS
Requires full range of body motion with the ability to bend, kneel and stoop. Must possess:, the ability to sit and or stand for long periods of time, the ability to lift approximately 20 lbs., visual acuity with color vision, the ability to communicate effectively and calmly in a professional manner, the ability to follow all workplace policies including safety and confidentiality, the ability to perform repetitive motion keyboarding functions, requires frequent exposure to communicable disease, toxic substances, ionizing radiation, medicinal preparations and other conditions common to clinic environment. Knowledge of the professional medical practice and ability to safely evaluate and provide health care to patients. Knowledge of organizational policies, regulations, and procedures to administer patient care. Must possess the ability to work with and around medical equipment and instruments safely and effectively. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in preparing and maintaining records, writing reports, and responding to correspondence. Skill in developing and maintaining department quality assurance. Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures.
