What are the responsibilities and job description for the Psychiatric Mental Health Nurse Practitioner position at Katahdin Valley Health Center?
POSITION SUMMARY
The Psychiatric Mental Health Nurse Practitioner will work with KVHC patients in providing outpatient community mental health services. The Psychiatric Mental Health Nurse Practitioner will collaborate with the Primary Care Providers to recognize, treat and manage mental health and psychosocial issues while acting as a contributing member to the Integrated Primary Care Team.
RESPONSIBILITIES
1. Provides focused intervention for patients who are in need of mental health services.
2. Provide counseling, rehabilitation and support services.
3. Talk, listen, observe and analyze patients and suggest a suitable mode of treatment. They may refer to specialists for treatment.
4. Suggest therapy in groups or individually as a mode of treatment.
5. Provides feedback to the Primary Care Provider via documentation in Medical Record.
6. Initiates follow up to determine any changes in treatment approach.
7. Monitors and coordinates the delivery of behavioral health services.
8. Provides complete documentation of visit and the patients progress in the medical record.
9. Maintain up to date records and billing practices in a timely manner within KVHC’s protocols.
10. Works where indicated to effect behavioral changes in patients with or at risk for physical disorders.
11. Assist patients in need of developing daily and community living skills.
12. Provides patients with self management skills and education information needed so they can participate in their treatment.
13. Provides consultation with KVHC Management and other clinical team members about behavioral services and suggested areas of program evaluation.
14. Assists patients with complying with medical treatment such as offering strategies to cope with medication side effects or chronic pain.
15. Provides case management functions linking patients to additional services.
16. Work closely with psychiatrists, psychologists, clinical social workers, psychiatric nurses and other mental health professionals.
17. Reports as required to the QI/QA Committee.
18. Perform other duties, projects as assigned by the Medical Director.
19. Adheres to all KVHC policies and protocols.
20. Complies with all safety rules and protocols. Reports all incidents, regardless of severity, immediately to their supervisor and completes an incident report and investigation form, which will be forwarded to Human Resources within 24 hours of the incident.
21. Conducts themselves as a good steward in the communities served by KVHC; represents the organization at outreach activities as needed.
22. Actively participates as committee member as assigned by the Chief Executive Officer.
EDUCATION AND EXPERIENCE
1. Masters Degree from an accredited college or university in Psychology, Social Work or Human Services.
2. Licensed Clinical Social Worker or Licensed Clinical Professional Counselor.
3. Must possess the ability to be credentialed and covered by FTCA Malpractice Coverage.
4. Working knowledge of behavioral medicine and evidence based treatment for mental health issues.
5. Has the ability to make accurate clinical assessments of mental and behavioral conditions.
6. Must be able to work in primary care as part of an integrated team.
7. Must possess strong communications skills.
8. Must possess the ability to design and implement clinical pathways and protocols for treatment of chronic conditions.
9. Strong knowledge of various social, cultural and work place environments.
10. Must possess a clean criminal and DHHS background.
11. Must possess the ability and means to travel between clinics as needed.
12. Valid State of Maine Driver’s license and ability to be insured by KVHC’s vehicle insurance carrier.
