- Provide intake and referral for incoming clientele
- Collect from and assist the clientele in obtaining the necessary documentation
- Identify clients needs, determine appropriate action, and erfer appropriate services
- Provide follow-up on referals made
- Prepare, maintain, and file reports, forms and records.
- Date entry into the H-HEAP database program and Shah client database system
