OVERVIEW: As a Memory Care Nurse, you will be an integral part of our healthcare team, providing compassionate and comprehensive nursing care to our tenants. QUALIFICATIONS: Active Licensed Practical Nurse (LPN) license in the state of Ohio.
Prior experience in skilled nursing or senior housing is preferred
Strong clinical skills with a focus on geriatric care
Excellent communication and interpersonal skills
Ability to work effectively within a multidisciplinary team Compassionate and patient-centered approach to care
DUTIES AND RESPONSIBILITES:
Provide nursing care and support to tenants in the assisted living community Administer medications and treatments according to physician orders and community protocols
Monitor tenants health status and respond promptly to any changes or concerns Coordinate with other healthcare professionals, including physicians, therapists, and caregivers, to ensure holistic care for tenants
Educate tenants and their families on health conditions, treatment plans, and preventive care measures Document all care provided accurately and maintain up-to-date tenants records
Participate in staff meetings, training sessions, and quality improvement initiatives Adhere to all regulatory requirements and maintain compliance with state and federal guidelines
