We are seeking a Home Health RN to support our client on across the state Pennsylvania! You will be responsible for the assessment, diagnosis, and treatment of assigned patients and have the ability to choose which shift.

Our client is seeking a compassionate and experienced RN to join their extraordinary care team.

This full-time position involves providing management and delivery of patient care plans in Irwin, PA and surrounding areas.

The role requires on-call availability on weekends and local travel to patient locations. The successful candidate will have an associate degree, an RN license in the state of work, and at least one year of RN experience.

Key Responsibilities ● Lead and collaborate to provide management and delivery of patient care plans ● Support patients in Carnegie/Crafton, PA and surrounding areas ● Spend ~20% of the day driving to/from patient locations ● Consistently meet critical deadlines for charting ● Self-manage time and schedule Schedule and Shift Details ● On-call Friday, Saturday, and Sunday from 5:00pm to 10:00pm 8 units on Saturday 8 units on Sunday, 2 holidays per year Travel ● Local travel requirements

Additional Responsibilities:

• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Qualifications:​​​​​​

• Associate degree or higher

• 1 years of experience in ANY of the following:

  • Clinical Documentation

  • Clinical Informatics

  • Clinical Quality

• 1 years of experience in ANY of the following:

  • Electronic Medical Record (EMR)

  • Other EMR

  • Practice Partner EMR

• Valid Registered Nurse (RN) in the state of Pennsylvania (PA)

• ALL of the following valid licenses/certifications:

  • American Red Cross Advanced Life Support (ARC ALS)

  • American Heart Association Advanced Cardiac Life Support (AHA ACLS)
