The PPS/MDS Coordinator oversees timely completion and submission of the required interdisciplinary assessments in skilled nursing and acute rehabilitation. The PPS/MDS Coordinator works closely with the patient/resident and with the entire treatment team to coordinate an accurate assessment and to facilitate appropriate care planning and intervention.

Education
Preferred: BSN, MSN, or currently enrolled in RN-BSN program.
Experience
Preferred: Experience with standardized outcome measures such as the FIM.
Department Specific Requirements
For the Transitional Care setting: experience as a licensed nurse in providing direct nursing care to the aged, chronically ill, developmentally disabled, or disoriented persons in a long term facility preferred.
For acute rehabilitation: candidate should have at least 1-2 years clinical experience as an RN,OT, or PT, in an inpatient rehabilitation setting.

Monitors RUG category to assure classification matches each MDS if in Transitional Care services. Monitors case mix groupings for acute rehabilitation.
Monitors quality indicators from software system and reports to Director of Nursing. (Transitional Services only).
Monitors completion of required documentation by licensed staff and identifies problem areas. Facilitates team collaboration to ensure appropriate classification of patients. Provides education as needed to clinical staff regarding PPS requirements. Stays current with any regulatory changes related to PPS and educates staff.
Acts as a liaison with the billing department.

REHABILITATION:
Produces quarterly rehabilitation program evaluation report, including assessment of data related to customer satisfaction, functional outcomes, required data assessment for specialty accreditation in CVA and amputee, referral patterns. Presents report to PI committee, assists as required in sharing results at department staff meetings. Identifies training needs and opportunities for improvement in performance. Provides training in collaboration with leadership. Plans and implements improvement projects using PDCA model. Monitors for effectiveness and modifies as needed. Collaborates with nursing and therapy leadership to develop consensus regarding targeted improvements, priorities, time frames, measurement indicators and threshold for success. Works closely with PI liaison to incorporate best practices in performance improvement into approach. Serves as rehabilitation services quality expert.
