**Core Duties:**
The Family Nurse Practitioner (FNP) operates as an independent healthcare provider within a primary care team. This role requires a self-directed individual who enjoys a significant level of autonomy. The FNP is typically responsible for a defined group of adult and geriatric patients, aged 18 and older. They utilize health assessment tools and data to help patients access the appropriate level of care and available resources. As part of the Medical Treatment Facility (MTF) Care Team, the FNP coordinates patient care throughout the continuum, aiming for optimal outcomes in clinical effectiveness, quality, and cost-efficiency.
The FNP provides direct clinical care to patients needing mid-level provider interventions while also offering educational resources and medication guidance as necessary. They apply population health guidelines to ensure patients receive timely and relevant clinical preventive services. In addition, they review diagnoses and medical records for active-duty members to assess pre-deployment and post-deployment fitness and health issues. The FNP evaluates and responds to telephone consultations and MHS Genesis In-Between encounters from patients and colleagues. They also deliver risk factor counseling and preventive services, prioritizing identified risks and directing patients to Specialty Services, the Health and Wellness Center (HAWC), or other preventive programs when appropriate.
The FNP develops and uses protocols aligned with existing clinical guidelines and creates tools to monitor metrics, continually improving the effectiveness of programs and the overall health of their patient population. Additionally, they understand the scope of practice for support staff and provide opportunities for their personal and professional growth, guiding them to broaden their experience and enhance patient care delivery.
In a private setting, the FNP conducts medical assessments to evaluate deployment status. They perform pre-deployment reviews of health records to identify any conditions that could affect deployment eligibility. By reviewing the Electronic Health Record (EHR), they assess current prescribed medications and discuss the use of over-the-counter medications and supplements with each patient. The FNP also verifies that all medications align with Area of Responsibility (AOR) restrictions, initiates waivers when necessary, and informs Public Health and the Deployment Waiver Manager accordingly.
They provide health education and recommendations for medical referrals and follow-up regarding deployment-related health issues, ensuring compliance with applicable Clinic Practice Guidelines (CPGs). A complete and legible copy of Deployment Readiness Health Assessments (DRHAs) 1-5 must be documented within the deployer’s EHR.
The FNP ensures that Service Members receive comprehensive counseling on various contraception methods, pregnancy-related concerns, and menstrual suppression as per DHA-PI guidelines, prescribing specific deployment medications based on Public Health recommendations and the deployment location. They finalize deployment determinations and indicate clearance status in the ASIMS DMC module, updating the ASIMS DD Form 2766 in accordance with AFMAN 41-210.
When needed, the FNP coordinates referrals with the deployer’s Primary Care Manager (PCM) via T-CON, typically referring to the Internal Behavioral Health Consultant (IBHC) in Primary Care for initial mental health care when the Behavioral Health Optimization Program (BHOP) is available. Finally, they complete the Healthcare Provider section of medical waiver packages for deployment-limiting conditions following the BOMC standard work process and capture active component DRHA workload using the Medical Expense and Performance Reporting System code.
Work Schedule:
Monday - Friday, 9 hours between 7:00am and 5:00pm, including a one hour lunch
**Minimum Qualifications:**
- **Degree:** Master’s degree in Nursing.
- **Education:** Graduate from a nursing specialty academic program accredited by a recognized nursing accrediting agency (ACEN or CCNE).
- **Certification:** Certification as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC), the American Association of Nurse Practitioners (AANP), or another appropriate national certification organization.
- **Experience:** A minimum of one year of Nurse Practitioner experience following graduation.
- **Licensure:** Possession of a current, full, active, and unrestricted Registered Nurse license.
- **Life Support Certification:** Current certifications in Basic Life Support (BLS) for Healthcare Providers, issued by the American Red Cross (ARC) or the American Heart Association (AHA).
- **Security Clearance:** Ability to pass a Government background check and obtain security clearance.
