Position Summary
The Licensed Practical Nurse (LPN) is responsible for utilizing the nursing process to provide for optimum quality and continuity of nursing care to patients in the comfort of their home. This individual demonstrates the core nursing competencies established by the agency to ensure effective and efficient care on an ongoing basis.
Essential Job Functions, Responsibilities and Activities
- Adhere to, uphold, and champion the Agency's Values.
- Adheres to the scope of practice for a Licensed Practical Nurse per state regulatory guidelines.
- Demonstrates appropriate judgment skills to be able to make independent clinical decisions in routine patient care matters.
- Promote the level of clinical expertise required of staff to provide safe, high quality nursing care
- Remains flexible and responsive when changes occur in patient activity and workload
- Performs appropriate documentation to maintain the standards set by the Agency.
- Monitors visit frequency and duration by reviewing physicians’ orders and comparing to the plan of care and documentation.
- Fosters an environment for open communication and collaborative practice.
- Evaluates/assists in evaluation of appropriateness of plan of care and revise on an ongoing basis with communication to all other disciplines.
- Plans, teaches, supervises and counsels regarding physical care measures to promote improvement/recover, symptom/complication management, expected disease progression, social/emotional care and community resources
- Perform comprehensive assessments of physical, emotional, psychosocial, and environmental needs of the patient.
- Reviews results of laboratory tests and other diagnostic procedures.
- Develops and implements a realistic individualized plan of care (Physician’s Plan of Care, Home Health Aid Plan of Care) for each patient, evaluates and revises this plan as necessary.
- Participates in development of patient goals and discharge plans as well as makes revisions to these based on the changes in patient status.
- Prioritizes nursing visits based on the patient’s health status and anticipated needs.
- Demonstrates the skill and judgment necessary to implement physician orders, nursing procedures and nursing interventions.
- Participates in utilization review of medical records as assigned.
- Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
- Appropriately utilizes ICD-10 codes.
Required Experience, Qualifications, Knowledge, Skills and Abilities
Education
- Graduate of an accredited School of Nursing.
Licensure and/or Certification
- Must currently hold a current unencumbered LPN/LVN license through the Arizona State Board of Nursing.
- Must have current CPR and Fingerprint Clearance card.
- Valid driver's license with proof of insurance in accordance with state requirements. 100% travel required.
Experience
- A minimum of 3 years’ experience working as an LPN/LVN required.
- A minimum of 1 year experience in Home Health care required.
- Skills we seek are the ability to educate on multiple disease process, good medication knowledge, wound care experience, and experience with wound vacs.
- Flexibility with schedule and able work weekends and holidays is a plus.
