Government issued photo ID required. Photo Identification must be carried on person during hours of service to client.
Proof of Everify or Attestation
Please require if nurse is working on a compact license
Flu Vaccine required during flu season (Oct.1 - March 31). Facility specific exemption request are due 11 / 1, or 15 days from hire date. Exemptions are on a case by case basis. If exemption approved candidate must wear mask.
