JOB Description for Structured Day Program Specialist
We are looking for a full-time or part-time employee to join our Adult Structured Day Program team. The individual should have availability Mon-Fri. Candidate should have experience working with adults and/or elderly (could be family or friend/preferrable at least a year) individuals with mild disabilities/limitations.
Experience with TBI (Traumatic Brain Injury) preferred but not required. An art aptitude and/or background preferred, but not required. Current PCA preferred but not required.
Essential Duties and Responsibilities
Reliability, consistency, as well as, timely and accurate complete of all New Journeys Day Program paperwork directly reflecting daily interactions and care needs.
Adhere to the Health Insurance Portability and Accountability Act (HIPPA) privacy and policies and procedures.
Maintain confidentiality of all information pertaining to clients, families and employees.
Maintain positive working relationships with clients, client's families, New Journeys staff, community providers and referral sources.
Services and Procedures (May Include)-
Assist with all activities conducted in the day program; which could include:
Assisting with toileting/hygiene
Transfers and ambulation
Assistance with eating and/or meal preparation
Assisting, monitoring and/or prompting the client to complete tasks
Integral components of an individual's personal care plan often include redirection, problem solving, monitoring and observation.
Job Description for HAB/PSR Specialist position
We are looking for someone with experience working with Behavioral Health population. Kee to Independent Growth (KIGI) is interested in someone with compassionate and highly motivated to help those in need. This position is 1:1 and designed to assist individuals with a behavior health diagnosis (i.e. SUD or Mental Health) in acquiring, retaining and improving skills such as - communication, job readiness & job skill development, self-help, domestic, self-care, socialization, relationship development, use of community resources and adaptive skills necessary to reside successfully in home and community-based settings.
Qualifications
Must be at least 18 years of age and have a high school diploma or equivalent, and 1-3 years of relevant experience working with individuals with TBI or memory disorder.
