The Registered Nurse (RN) in Home Health provides nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.
Key Responsibilities:
- Coordinate total plan of care with other healthcare professionals involved in patient care
- Develop and implement individualized plans of care within state-specific guidelines
- Conduct comprehensive assessments and develop appropriate care plans
- Ensure patients meet home health eligibility and medical necessity guidelines
Requirements:
- Current RN licensure in the state of practice
