Job Summary: We are seeking a dedicated and compassionate Nurse Practitioner to join our team at a reputable substance treatment facility. The ideal candidate will have experience in addiction medicine, providing high-quality patient care, and supporting individuals on their journey to recovery. You will collaborate with an interdisciplinary team to create and implement personalized treatment plans for patients struggling with substance use disorders.
Key Responsibilities:
- Conduct comprehensive assessments, including medical, psychological, and social history evaluations.
- Develop and implement individualized treatment plans in collaboration with patients, families, and the treatment team.
- Monitor patient progress and adjust treatment plans as needed to ensure optimal outcomes.
- Administer and prescribe medications, including medication-assisted treatments (MAT), and monitor their effectiveness.
- Provide patient education on addiction, recovery, and overall health and wellness.
- Perform routine medical procedures, such as blood draws, injections, and wound care.
- Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated and comprehensive care.
- Document patient information, treatment plans, and progress in electronic health records (EHR) systems in a timely manner.
- Participate in team meetings, case conferences, and continuing education opportunities.
- Promote a supportive and empathetic environment that encourages patient engagement and trust.
- Completing peer to peers with insurance providers.
Qualifications:
- Current and unrestricted Nurse Practitioner license in the state of Texas.
- Master's or Doctorate degree in Nursing from an accredited institution.
- Certification as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) preferred.
- Minimum of 2 years of experience in addiction medicine or a related field.
- Strong understanding of substance use disorders and evidence-based treatment modalities.
- Excellent clinical assessment and diagnostic skills.
- Ability to work effectively in a team-oriented and fast-paced environment.
- Exceptional communication, interpersonal, and organizational skills.
