Job Summary

This position is responsible to plead and/or act on behalf of an individual elder or group to obtain a service. The Elder Advocate assists elders to obtain their rights and to obtain their objectives including: respite care, in-home chore services, and other activities as needed. The Elder Advocate will assist the elders that are in need of assistance with their instrumental activities of daily living (IADL's).

Essential Functions

  • Elder Advocate completes in-take assessment identifying need for service/resources available, for elder's general health, financial, transportation, isolation, intervention/abuse risk, nutrition, respite and other needs
  • Ongoing case management and collaborating with appropriate KIT/Community programs ensuring services obtained and ongoing needs are provided as planned
  • Schedules regular Tyotkas services i.e. Chore Service, home visits, shopping, regular transportation, respite care and additional services as supported by current grant/funding requirements
  • Elder Advocate works with families and refers elders to the Adult Protection Team, Tribal Court Team, Dena'ina Wellness Center, and ADRC, as needed and works as an integral part of the team providing follow up and referral
  • Elder Advocate is responsible for arranging germane training and information for elders such exercise, budgeting, estate planning, and partner with DWC to provide nutrition, exercise opportunities
  • Quarterly follow up calls to elders receiving services for quality assurance and improvement
  • Assist with IADL's as needed, limited chore service, meal preparation and eating assistance
  • Perform basic first aid procedures
  • Assist elders with obtaining medications as directed by a medical provider
  • Assist with medical appointments, motion & transfer, shopping, banking, paperwork, chore service
  • Mentor and assist the elder advocate(s) as needed to perform elder care duties
  • Provide documentation as required for coordinating services as directed
  • Assist in setting up elder functions and activities as needed
  • Vehicle related duties include purchasing fuel, arranging for maintenance, visual inspections, vehicle related janitorial and vehicle condition reports
  • Maintain records of transports provided
  • Respect Tribal equipment to include vehicles
  • Treat elders with respect and courtesy
  • Transportation service as needed
  • Reports incidents of suspected elder abuse and or neglect according to State law
  • Participates in Emergency Preparedness Plan
  • Maintains confidentiality in accordance with the Tribe's policy
  • Ensures compliance with Tribal, federal, state, and local employment laws and regulations, Tribal policies and TERO ordinance

Qualifications

Education

  • High School Diploma or General Education Degree (GED)

Experience

  • Experience working with elders and vulnerable adults

Preferred

  • Knowledge and experience working with cultural diversities

License/Certification

  • Must have a valid Alaska State driver's license and must remain insurable under the Tribe's policy, or obtain within 90 days of hire, or, with approval of the hiring manager, sign a *Memorandum of Understanding (MOU) in lieu of having a driver's license to perform the required duties of the position (If Applicable)
  • Current First Aid & CPR Certification, or within 90 days of hire
