The Program Specialist (PS) is responsible for overseeing KenCCID programs and activities, Individual Assessments and Individual Support Plans (ISP) developed for Individuals with Disabilities. This position is responsible for communicating and coordinating with the Residential Directors in the assessment, coordination, implementation, monitoring and evaluation of the ISPs in compliance with the licensing requirements set forth in Pennsylvania law (55 Pa. Code § 6100 and 6400, hereinafter “6100 and 6400 regulations”).
The intent of this description is to provide a representative summary of the major duties and responsibilities performed by incumbents of this job. Incumbents may be requested to perform job-related tasks other than those specifically presented in this description.
The PS position is a full-time salaried position. The PS works in conjunction with Residential Directors (RDs) and Community House Managers (CHMs) and reports to the Compliance Manager.
Core Responsibilities
- Ensures that the safety and well-being of the Individuals is protected at all times and that the rights of the Individuals are observed at all times.
- Assigned to a maximum of 30 Individuals.
- Coordinating the completion of assessments as per 6100 and 6400 regulations
- Leads in the process, development, team reviews and implementation of Individual Support Plans (ISP) in accordance with the 6100 and 6400 regulations.
- Monitors the implementation of ISPs and ISP outcomes as written.
- Ensures all individuals’ personalized documentation are updated annually in EHR including signing and dating.
Individual Support Plan (ISP) Responsibilities
- Coordinates and completes assessments as per 6100 and 6400 regulations requirements.
- Provides assessments as required under 6100 and 6400 regulations.
- Monitors assessment content and develops addendums updates.
- Participates in the development of the ISP, ISP annual update and ISP revision under 6100 and 6400 regulations
- Attends the ISP meetings
- Fulfills the role of plan leader, in relation to the development, annual update and revisions of the ISP; and ISP review
- Reviews the ISP, annual updates and revisions for content accuracy
- Reports content discrepancies to the Support Coordinator and plan team members, when applicable
- Completes ISP quarterly reviews as per 6100 and 6400 regulations
- Monitors and assures ISP is implemented as written.
Individual Services Responsibilities
- Supervises, monitors, and evaluates services provided to the Individual
- Review, approve, sign and date the monthly documentation of an Individual’s participation and progress toward outcomes[RO1]
- Developing person-centered plans, locating, coordinating, and monitoring services and Family Driven supports for individuals with intellectual disabilities[RO2]
- Connect with community resources and specialized services[RO3]
- Leads the KenCCID members of the Individual Plan Team to ensure Residential Needs Assessments and Individual Service Plans are updated in compliance with Applicable Regulations, and that internal care providers are promptly trained on modifications[RO4] ..
- Ensures that Individuals who are working meet all applicable earning limitations and restrictions under the Ticket to Work Act, and makes all necessary reports to Social Security, Medicaid, and any other agency.
- Report a change related to the Individual’s needs to the Program Director, Compliance manager, Support Coordinator, and plan team members when applicable.
- Review and show accuracy of the ISP with the Individual as required.
- Provide ISP review documentation to the Support Coordinator, as applicable, and plan team members; and make applicable recommendations for revisions
- Conduct training for Community House Manager and Direct Support Professionals (DSPs) on approved ISP[MWA5]
- Inform plan team members of the option to decline the ISP review documentation as required under the 6400 regulations
- Oversee the activities and coordinate the services provided to an Individual[MWA6]
- Trains RDs and CHM on the assessment, ISP and ISP outcomes methodologies
- Coordinate the training of DSPs in the content of health and safety needs relevant to each Individual[MWA7]
- Develop and implement provider services as required
- Performs other duties as required

Compliance Management of services to the Individual[MWA8]
- Maintains up-to-date knowledge of the 6100 and 6400 regulations, including bulletins and other applicable DHS-issued publications.
- Routinely conduct audits under 6400 15 (c) specific to the Individual, services, medical and physical site.
- Ensures ongoing compliance with regulatory plans of correction; ODP licensing, QA&I, Provider Monitoring, and other quality concerns to the individuals.
- In the Certified Investigator role, conducts investigations, including but not limited to allegations of abuse and neglect, on an as-need basis.
- Ensures service notes report is run daily and communicate any discrepancies to compliance Manager, Residential Directors, Program Director, EHR Director.
- Ensures that all individual’s behavioral data is collected and uploaded in EHR monthly including behavioral support notes.
Education and Certifications
- Master’s degree from an accredited college or university and two years of related work experience, working directly with Individuals with disabilities; or bachelor’s degree from an accredited college or university and three years of related work experience working directly with Individuals with disabilities; or associate degree or 60 credit hours from an accredited college or university and five years of related work experience working directly with Individuals with disabilities.
- Current certification as ODP Certified Investigator, or eligible to obtain such certification within one year, or sooner subject to course offering.
