  • Required Certifications : BLS
  • State License Requirements : ARRT, ARDMS

    • Additional Details

  • Proof of vaccines or titers : MMR
  • Proof of vaccines or declination : Hepatitis B
  • Influenza Vaccine : October 1 – March 31 (accepts CHS Declination)
  • TB Test : One Step Tb Skin Test / Tspot / TB QuantiFERON within past 12 months prior to start date
  • Proof of vaccines or titers : Varicella
  • Annual TB Questionnaire : Within past 12 months
  • Physical / Health Statement : Physical and Vision Exam within the past 12 months prior to start date
  • Catholic Drug Screen : 12 panel (NO THC) in the past 12 months prior to start date
  • Catholic Criminal Background Check : 7-year Criminal County Search, SSN Trace, National Criminal Database within 12 months prior to start date
  • Catholic National Sex Offender Registry : Within 30 days prior to start
  • Catholic State Sex Offender Registry : Within 30 days prior to start for the state that the talent lives in.
  • Catholic Webchecks : Within 30 days prior to start
  • Catholic Resume : Required for submission
  • Catholic Professional Reference : Required
