Our RN Case Managers’ duties are designed to help you succeed in providing one on one patient care in the security of our patient’s home.
- Responsible for patient assessment, care delivery and coordination, communication and collaboration with patient’s physician and KVNA’s interdisciplinary team – PT OT ST
- Health promotion by teaching and training family members on proper home care procedures and strategies
- Develop home health aide plans of care and provide direct supervision

Requirements
- Valid WI RN License
- Current CPR certification
- Med/Surg or ER experience preferred
- Active Driver's License and reliable insured transportation
- Drug screen and background check
