GENERAL DESCRIPTION:
The Community Offsite Case Manager reports directly to the Program Director and works in close partnership with KSI Management Team. Under the direction of the Program Director, this position maintains regular contact with participants employed at offsite KSI contracted work programs and who may additionally attend KSI programs. Also responsible for maintaining caseload, following all applicable rules and regulations. Supports and empowers participants to assist in securing and maintaining employment opportunities through coordination with KSI Community-Based Operations. Serves as a liaison between offsite work employers, participants, KSI Community-Based Operations, and all KSI staff. May have to work outside normal KSI business hours to coordinate transportation. Provides training and supervision of Direct Support Professionals as needed.
This position is a full-time position, Monday – Friday 8:00 am to 4:00 pm, pay grade 14 with a starting annual salary of $45,000.00. ESSENTIAL FUNCTIONS:
Provides specific programming for each participant on caseload to ensure individual plans are implemented with regard to timeframes; maintains caseload and accurately tracks/monitors participant information, individualized plans, and case files per required timeframes; develops participant service plans and completes required documentation to ensure program compliance; conducts required routine assessments, care plans meetings, data collection, and quarterly and yearly reviews in electronic database management systems within required timeframes; maintains knowledge of wages and benefits requirements to assist participants in making informed choices for work incentives and to achieve self-sufficiency through work; ensures participants wages are accurate, coded, and distributed; coordinates and monitors participants work and transportation schedules to offsite KSI contracted work programs, including participants weekly attendance to other KSI program sites’ develops professional working relationship with participants and families; provides coverage and support during program activities and acts as a supervisor as needed for area coverage. Must maintain specific training and/or certification applicable to the position as required by DDDS, MANDT and CPR/First Aid/AED.
SKILLS AND ABILITIES:
Must possess excellent written and oral communication skills for creation of individual support plans, programs, reports, correspondence and other documents. Must have established problem solving and conflict management skills; have knowledge and experience in and/or relevant experience in working with individuals with intellectual and/or developmental disabilities; must have efficient interpersonal skills.
QUALIFICATIONS:
A Bachelor’s Degree in Human Services, Behavioral Health, or other related field is preferred. Two years of relevant case management experience is required. Supervisory experience is required. Must be able to pass a criminal background, adult and child abuse registry check and sex offender listing. Must have a valid Delaware Driver’s license and have no more than two moving violations at the time of hire.
