Registered Nurse Assessment Coordinator (RNAC) - Weekend Position
Kentmere Rehabilitation and Skilled Nursing, a 104-bed skilled nursing facility, is seeking a Part-time Weekend RNAC to join our team!
Located on the historic outskirts of Wilmington’s forty-acre neighborhood, blocks from Trolley Square and Brandywine Park, Kentmere is easily accessible to routes 95, 202 and 52.

Job Duties:
- Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of RAP's and Triggers
- Maintain, and periodically update, written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
- Participate in facility surveys made by authorized government agencies
- Evaluate each resident's condition and pertinent medical data to determine needs
- Contact, and assist in scheduling, participation by members of the care plan team, including the resident's representative and/or other interested family members
- Develop preliminary and comprehensive assessment of the nursing needs of each resident, utilizing the forms required
- Ensure that the assessment team is aware of the importance of completeness and accuracy in their assessment function and ensure awareness of penalties
- Assist in monitoring Quality Measures
- TSI Scrubber for MDS’s
Qualifications:
- Licensed RN in the state of DE
- RAC-CT Certified or Working towards Certification
- 3 years of MDS/ RNAC experience in a long-term care setting required
- Managed care and bundled payment knowledge
- PDPM and Medicare A Management experience required
- Possess excellent oral and written communication skills with computer literacy, required
- Extensive knowledge of state and federal regulations and state survey process, required
