CASE MANAGER / PDS SERVICE ADVISOR :
The Kentucky River Area Development District, an eight-county planning and development agency, is accepting applications for a Case Manager / Service Advisor. This position provides coordination of Participant Directed Services (PDS).
Examples of duties include but are not limited to :
Develop a plan of care and case management for Medicaid clients that are at risk of being institutionalized.
Minimum Qualifications :
Three years human services related experience; An Associate degree in Social Work, Health, or Human Services field and at least one-year experience in a health or human services field or the educational or experiential equivalent in the fields of aging or disabilities; or a Bachelor degree in Social work, health or human services fields from an accredited college or university; or a Registered nurse who has a current Kentucky license with at least two years’ experience working with the elderly or disabilities Master’s Degree in Social Work, Health, or Human Services field OR fifteen years’ experience working in a Human Services related field. Education and experience will denote salary tier.

Applicants must have good working knowledge of computers and software applications and possess a valid driver’s license and dependable vehicle. Possesses good written and oral communication skills; knowledge of reporting and vital record keeping requirements; ability to establish and maintain effective working relationships with other employees and clients who could have mental or criminal backgrounds. A competency test and criminal records check will be required before interviewing with the Personnel Committee of the KRADD. Applicants must pass a drug screen for employment offer.
