What are the responsibilities and job description for the Patient Care Coordinator position at KentuckyCare?
POSITION SUMMARY
The Patient Care Coordinator (PCC) will be responsible for ensuring that patients receive comprehensive,
coordinated care throughout their healthcare journey. The PCC will work closely with patients, their
families, healthcare providers, and other stakeholders to facilitate communication, manage care plans,
and provide necessary resources, referrals, communication, behavioral health, coordinated care, and
insurance.
- Patient Support and Coordination
- Develop and manage personalized care plans for patients, addressing their unique needs and
preferences.
- -Serve as the primary point of contact for patients and their families, providing guidance and

- -Monitor patient progress and adherence to care plans, making adjustments as necessary to
- -Coordinate referrals to specialist, behavioral health services, and community resources.

SKILLS & ABILITIES
Education: High School Diploma or General Education Degree (GED)
Experience: Experience in customer service, healthcare, or related field required.
Computer Skills: Use of electronic medical record and other information technology will be required.
