PRSC –Psychiatric Rehabilitation Services Coordinator

Kenwood Village, a skilled-psych facility located in Chicago, Illinois currently has a full time position for a PRSC.
Position Summary: To provide social service needs to adults in a long term care/nursing home setting.
This position requires
1. BA/BS, MA/MS REQUIRED
2. Flexible hours working weekend rotations
3. Must be able to work well within the context of a Long-Term Care facility.
4. Demonstrates good judgment under stress, emotional stability, tact, reliability and ability to adapt changing circumstances
Other responsibilities include:
1. Providing the resident with a stable therapeutic relationship
2. Orienting the resident to the facility.
3. Reviewing and assisting the resident in understanding his/her treatment plan and program schedule i.e. day program, in-house groups and activities.
4. Preparing and assisting the resident with active participation in his/her treatment plan review.
5. Providing and/or coordinating the delivery of the psychiatric rehabilitation services programs
6. Monitoring the resident in the areas of self-directed care and for overall compliance with their treatment plan.
7. Completing proper documentation upon admission, quarterly, and annually.
8. Documenting the resident’s progress on a regular basis, including incidental, monthly and quarterly notes.
9. Contacting community agencies for the purpose of resident referral.
10. Maintaining knowledge of residents on psychotropic medications, including the identified purpose of a specific medication and participating in behavioral programming to help the resident achieve his/her highest functional level.
11. Lending group and individual counseling sessions, as assigned.
12. Being familiar with the facility policies on behavioral emergencies and be trained in crisis management and diffusing a potentially volatile situation, especially concerning aggressive/agitated behavior.
13. Participating in educational training's and in-services provided by the facility.
14. All other duties as assigned.
