Funded through The Center for Rural and Primary Healthcare, the Library Community Health Worker will connect residents to health and medical care, insurance, and other social services. The position will maintain cases through an interactive portal and provide necessary grant reporting. Developing and maintaining close working relationships with community organizations, health providers, and treatment centers will be an important responsibility of this position, which also includes maintaining a resource directory of community support.
The Community Health Worker is also responsible for interacting with and guiding individuals, their families, and/or supportive partners to better understand, navigate, and access community services. This will be done by connecting individuals with appropriate resources and working to implement healthy behavior change.
To support the health and well-being of the community as a whole, the Community Health Worker will collaborate with library staff, community organizations, and other partners to provide health-related programs. The position will report to the Library Director.
The Community Health Worker will:
- Effectively demonstrate enthusiastic, outgoing, and client-service oriented characteristics.
- Pursue certification through the SC Community Health Worker Association.
- Assist customers in accessing community services, in filling out applications, and in understanding information from health organizations in relation to Medicare, Medicaid, Benefit Bank, Affordable Care Act benefits, and Social Security benefits.
- Assist customers in accessing community services, complete applications and understand information from social service organizations.
- Assist marginalized, at-risk, and all customers in finding solutions and resources to address social, behavioral, and educational needs.
- Research and refer clients to medical, health and community resources. Work with individuals, families or groups.Respond to clients in crisis situations.
- Create a low-barrier entry for individuals seeking social work services.
- Provide case management to clients dealing with issues affecting their physical, emotional and social well-being.
- Responsible for grant reporting and keeping accurate and timely documentation to support grant outcomes.
- Maintain accurate records and documentation of client interactions.
- Serve as a liaison for community partnerships with social service agencies
Education / Experience Requirements:
An accredited associate degree or higher in a field such as business, human services, or a related field. Work experience in fields related to the job, such as education, library work, social work, public health, and health education.
