We are seeking a compassionate and skilled Program Director to join our team and provide and oversee high-quality in-home care and support to individuals with intellectual and/or developmental disabilities. The Program Director will empower the individuals we support to excel to their fullest potential in all aspects of their life. The successful candidate will have strong management, communication and problem-solving skills and be able to work collaboratively with support teams and other team members. The Program Director will be dedicated and reliable, and will ensure the health, safety and on-going growth of the individuals at all times.




Job Duties & Responsibilities

  • Deliver needs-based support according to individual plans
  • Ensure consistent, person-centered support and individualized programming

  • Assist with daily activities, such as bathing, dressing, and grooming (as needed)
  • Provide or assist with transportation to activities, appointments, etc., as needed

  • Participate in recreational activities, including outings and community events

  • Complete all required shift documentation including, but not limited to, Daily Log Notes, Outcomes, MARs, Heath Needs Records, Provisions of Care, Incident Reports, etc.
  • Maintain accurate and up-to-date records of care provided and report any changes in the individual's condition to the appropriate team members

  • Collaborate with other team members to ensure continuity of care and services

  • Support the administrative team in the daily operations of the organization
  • Provide regular oversight and support to team members
  • Abide by all organization policies and procedures and all DHS requirements under MN 245D
  • Other duties as assigned

  • Provide a minimum of 32 hours of direct support per week following DSP Job Duties & Responsibilities, and a minimum of 4 hours of administrative hours
  • Be available for emergency on-call support if and when necessary
  • Abide by all organization policies and procedures and all DHS requirements under MN 245D


Job Qualifications

Required:

  • Successful completion of DHS background study
  • Minimum Education/Experience:
    • A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
    • An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older;
    • A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or
    • A minimum of 50 hours of education and training related to human services and disabilities; and
    • Four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
  • Direct experience in Home and Community-Based Services licensed under MN Statutes, Chapter 245D
  • Valid driver’s license and current vehicle insurance
  • Satisfactory Motor Vehicle Report (MVR) or driving record
  • Strong decision making, problem solving and judgment skills
  • Ability to multi-task and maintain organization
  • Ability to remain objective in working with team members and other situations
  • Technology and computer literate
  • Effective communication skills (written and oral)
  • Ability to read, write and speak English
  • Willingness to take initiative and adapt to circumstances
  • Experience working with individuals with dual diagnosis
  • Experience working with individuals across cultures
  • Excellent interpersonal and communication skills
  • Ability to adhere to scheduling requirements
  • Passion for making a positive impact in the lives of others


Preferred:

  • Current CPR/First Aid certification


Job Demands and Conditions

  • Must be willing to travel throughout the Twin Cities and surrounding areas
  • All working hours must be completed at the main office or at a service site
  • Any remote working hours must receive prior approval
  • Must stay with service recipient until arrival of team member replacement - when applicable
  • Shifts may require being on-call for support as needed
  • Shifts may vary including morning, day and evening
  • Shifts may include sitting, standing, or walking for extended periods of time
  • Shifts may involve actively engaging with the individual in their preferred activities (drawing, chasing, playing catch, etc.)
  • Shifts may include routine cleaning tasks and home maintenance responsibilities
  • Shifts may involve regular travel in the community using personal vehicle
  • Shifts may involve regular contact with service recipients, staff, family members and the general public
  • May be subject to physically aggressive and/or emotionally upset individuals
  • Must be able to bend and lift up to 50 pounds
