What are the responsibilities and job description for the In-Home Care Coordinator position at Key Elements Care, LLC?
Key Elements care is seeking RNs to provide oversight of patients and aides in the home setting. RN will provide professional care to the client, as indicated by Agency policies, State and Federal laws/ regulations which includes monthly visits, reassessments and annual reassessmens of clients. In-home, admission visits and eligibility determinations are part of the RN's duties, as is the development of the Care Plan. The RN also prepares paperwork necessary for Agency policies and federal/state laws and assists with the training and supervision of Home Care Aides.
Job Types: Part-time, PRN, Contract
Pay: $50.00 - $100.00 per hour
