Job Summary:

Lead Case Manager will oversee and coordinate services for individuals or families in need of support, ensuring that they receive comprehensive care and assistance tailored to their unique needs. This role typically involves supervising a team of case managers, developing, and implementing care plans, and ensuring effective service delivery.

RESPONSIBILITIES:

  • Supervise and mentor a team of case managers, providing guidance and support.
  • Develop, implement, and monitor individualized care plans to address clients' physical, emotional, and social needs.
  • Serve as a point of contact for clients and families, advocating for their needs and ensuring access to necessary resources.
  • Collaborate with healthcare providers, community organizations, and other stakeholders to ensure coordinated care.
  • Maintain accurate and uptodate case files, documentation, and reports in compliance with legal and regulatory standards.
  • Assess client progress and modify care plans as needed, ensuring continuous improvement and optimal outcomes.
  • Lead case review meetings and contribute to program development, quality improvement initiatives, and staff training.
  • Ensure the team adheres to policies, procedures, and ethical standards in service delivery.
  • Manage crisis situations and provide support during critical incidents.

EDUCATION AND EXPERIENCE:

  • Bachelor’s degree in social work, Psychology, Sociology Nursing or Human Services.
  • 35 years of experience working in case management, social services, healthcare, or related field.
  • . Experience in leading or supervising teams of case managers or other support staff is usually required, especially for a lead.
  • Skills in mentoring, training, and guiding junior staff are crucial.
  • Experience in healthcare, with a focus on patient care, medical case management or coordination.
  • Experience in helping clients with social services, housing, and addiction treatment.
  • Experience working with patients with SUD and cooccurring disabilities.
  • Requires a valid Washington state driver’s license and maintain active auto insurance on personal vehicle.
  • Computer and EHR literate.

ESSENTIAL SKILLS AND FUNCTIONS:

  • Reliable work attendance and ability to work evenings and weekend hours.
  • Demonstrated ability to establish and maintain a variety of records, organize data, prepare reports.
  • Knowledge of community resources and demonstrate ability to work cooperatively and responsively with them.
  • Ability to demonstrate Diversity, Equity, and Inclusion in all workrelated activities.
  • Ability to advocate for inclusion, equity and belonging for all patients and colleagues.
  • Ability to communicate effectively both in writing and orally.
  • Ability to plan, prioritize and organize your own workload and work independently.
  • Establish and maintain effective work relationships and maintain strong boundaries with clients.
  • Ability to teach life skills, such as budgeting and to facilitate group meetings.

  • Demonstrated ability to maintain current, accurate and complete documentation in patient records.
  • Ability to maintain confidentiality with sensitive information in accordance with HIPAA, 42 CFR and 45 CFR.
  • No history of misuse of alcohol or other drugs for a minimum of one year prior to employment with Key Recovery Center. Employer is a drugfree workplace, and employees will agree to preemployment and reasonable cause drug testing policies.

PHYSICAL DEMANDS:

The duties require sitting, standing, walking, stooping, bending, smiling, kneeling, reaching, pushing, pulling, twisting, and the ability to grip with hands and fingers. Ability to hear and speak conversationally, fluent in English occasionally lifting to 25 pounds. Must be able to climb stairs without an elevator.
