Keystone Autism Services is seeking a Supports Coordinator (Case Manager) to join our team in making a positive difference in the lives of the individuals we support.
Responsibilities Include:
- Administrative role that assists in coordinating supports and services utilized by adults living with autism
- Organize team meetings to develop and review Individual Service Plans
- Maintain database, documentation and case management files
- Liaison between outside supports, individuals, families and team members
- Assist individuals in gaining access to authorized services, as well as medical, social, educational and other services

Minimum Requirements:
- Bachelor’s degree in Psychology, Education, Social Work or other related Human Services or Social Sciences field
- Experience working with individuals living with autism and/or linking individuals to community based supports
- Minimum of 1 year experience working in a case management and/or supports coordination role
- Valid driver’s license and daily access to a properly insured vehicle
- Successful completion of the pre-employment process, including motor vehicle record and criminal background checks
