Overview:
We are seeking a dedicated and experienced Memory Care Coordinator to create, plan, and oversee the activity programming for our memory care residents. The ideal candidate will have a strong background in resident care, dementia care, and home care, with a passion for ensuring the well-being of our residents.
Duties:
1. Coordinate and provide meaningful activities that increase self-esteem, with sufficient support and supervision from staff throughout each day of the week. Examples of these structured activities include, but are not limited to: reminiscence, exercise, art projects, stimulating mind games, music, laundry folding, singing, baking, walking, religious services, outings, picnics, entertainment, etc.
2. Prepare monthly calendar, in conjunction with Life Enrichment Director, and post for residents, their families and staff, on the bulletin board; daily calendar is written in large print on a dry erase board in the main dining room.
3. Invite Families regularly to participate in special programs with their family member.
4. Assist in training and supervision, in conjunction with Health and Wellness Director, of personal care manager staff, who provide direct care to the memory care residents.
5. Work with residents' families to ensure that the residents are receiving the care and support that they need.
6. Assists in the coordination of the move-ins of new Residents and orientation to ensure satisfactory integration into the Community.
7. Shares in the responsibility for providing and maintaining an environment that is conducive to the safety and the physical and emotional well-being of Residents and personnel.
8. Documents Resident incidents appropriately, and follows up as necessary.
9. Demonstrates a true appreciation of the heritage, values and wisdom of the Residents and an understanding of the aging process.
10. Demonstrates the ability to function as a team member in sharing responsibilities for the administration of Resident services in cooperation with all members of the team.
11. Promotes and facilitates interactive problem solving with staff.
12. Performs other duties as assigned by Life Enrichment Director and/or Health & Wellness Director.
Qualifications/Specifications:
Education/Experience: The Program Coordinator must be at least 21 years of age, have a minimum of two years’ experience working with elders or disabled individuals, have knowledge of aging and disability issues, have demonstrated experience in administration and demonstrated supervisory and management skills. Previous experience in assisted living preferred.
Complexity: Ability to plan and prioritize work duties. Must be able to prioritize Residents' needs. Must be self-directed and industrious.
Supervision: Under the direct supervision of the Life Enrichment Director and works in conjunction with Health & Wellness Director. Must be able to follow through without close supervision.
Other Contacts: Constant contact with Resident, potential Residents, their families, and various health care providers. Must be courteous with all contacts.
Confidentiality: Will be exposed to highly confidential information, the disclosure of which would be contrary to the best interest of the Residents, the Community and professional ethics.
Physical Demands: The ability to perform high levels of physical activity for extended periods of time. Must be able to lift/push up to 50 pounds multiple times during a shift. Physical dexterity required in lifting, pulling, pushing, carrying, stretching, moving, bending and stooping. May sit or stand for extended periods of time.
Working Conditions: Clean, well-lighted household environment. Will be constant activity and frequent interruptions. Exposure to body fluids, infection, odors, and behaviors of Residents. Exposure to chemical/cleaning solutions.
Safety: Must utilize Universal Precautions when assisting Residents and/or handling any body fluids.
