Job Description:
Keystone Agricultural Worker Program (AWP) Southeast Region is seeking a dedicated and compassionate part-time Bilingual Case Manager to join our team. The Bilingual Case Manager will play a crucial role in assessing and addressing the social determinants of health (SDOH) for migrant and seasonal farm workers. This position involves providing health education and support to farm workers at their homes, farms, labor camps, and other outreach settings.
Key Responsibilities:
- Screen, interview, and assess farmworkers for SDOH disparities.
- Refer and enroll patients in appropriate services.
- Arrange follow-up consultations for provider-placed referrals.
- Assist patients with hospital-based charity care program applications and enrollment.
- Collaborate with outreach coordinators at monthly meetings.
- Cover front desk operations during clinic hours, including greeting patients, check-in, privacy notices, and payment collection.
- Attend weekly regional staff meetings.
- Accompany multidisciplinary teams on outreach trips to farmworker housing as needed.
- Act as an in-the-field social worker for SDOH-related questions.
- Assist with registration and screening of farm workers.
- Provide non-judgmental, linguistically, and culturally appropriate health education on topics such as chronic disease prevention, diabetes, hypertension, alcohol and drug abuse, mental health, depression, sexually transmitted infections, immunizations, and cancer screening.
- Register farm workers eligible for the Keystone Agricultural Worker Program.
- Assist the regional coordinator in identifying farms employing migrant and seasonal farm workers.
- Offer counseling services as licensure permits.
Qualifications:
- Bachelor's Degree in Social Work is strongly preferred, or a degree in a relevant field with adequate experience in social work.
- Must be able to read, write, and speak Spanish fluently.
