No holidays or weekends. No on call time.
Job Description:
We are looking for a dedicated RN Care Manager to join the healthcare team at Keystone Family Medicine. The RN Care Manager will provide comprehensive care management services to patients by developing, monitoring, and evaluating interdisciplinary care plans. The RN Care Manager will work collaboratively with the providers and care team to deliver patient centered care with a focus on education, self-management, and timely healthcare delivery to achieve optimal health outcomes. This role requires a professional who consistently demonstrates and promotes the values of respect, honesty, care, and dignity for patients and all members of the healthcare team.
Key Responsibilities:
- Develop, monitor, and evaluate interdisciplinary care plans for patients.
- Commit to the constant pursuit of excellence in improving the health status of patients and the community.
- Collaborate with the healthcare team, specialists, and patients/families to ensure the delivery of quality, efficient, and cost-effective healthcare services.
Qualifications:
- Associate degree in Nursing required; Bachelor's degree preferred.
- Minimum of three years of experience working in a clinical setting as an RN is required.
- Experience in care management desired.
- Current CPR certification.
- Current Pennsylvania RN License
- Strong interpersonal skills for effective communication with patients and healthcare team members.
- Ability to demonstrate independent judgment and strong critical thinking skills both in person and virtually, and to collaborate with the care team members.
