Primary Job Responsibilities
- Coordinates with third-party review service to ensure coding, OASIS Reviews, and POC Reviews are completed. Responds to any review holds and ensures timely resolution
- Collaborate with clinical staff to develop and implement quality improvement initiatives
- Provide education and training to nursing staff on quality assurance processes, best practices, and regulatory requirements
- Assist in the development and revision of policies and procedures to improve patient care quality
- Stays current on changes in regulatory guidelines related to the Home Health and Hospice industry
- Maintains proficiency in OASIS and Coding guidelines
- Manages and monitors patients in Transferred/On-Hold status. Ensures proper documentation of transfer and eventual discharge or ROC
- Access Hospital EMR records to obtain additional information needed for SOC, ROC, Transfer and Discharge as needed
- Reviews and closes all Transfer, Recert, ROC, and Discharge OASIS
- Regularly monitors active wound list and ensures proper wound scores, treatments, orders, and weekly photos are documented. Triggers clinical supervision to respond to any non-healing or degrading wounds
- Monitors list of patients classified as high risk for ACH. Participates in weekly IDT case conferences
- Serves as a backup clinical resource for the intake department as needed
- Participates in chart audit processes on occasion
- Attends virtual and on-site meetings as needed
- Registered Nurse (RN) with a valid Pennsylvania nursing license
- Current certification in OASIS and ICD Coding
- Minimum of three years of clinical nursing experience, preferably in home health or hospice
- Experience in quality assurance, compliance, or related field preferred
- Strong analytical skills with the ability to interpret data and trends
- Excellent communication skills, both verbal and written
- Detail-oriented with a strong commitment to patient safety and quality care
- Ability to work collaboratively within a team environment
- Proficient in Microsoft Office Suite and electronic medical records systems
- Proof of COVID-19 Vaccination or an approved Medical or Religious exemption
- Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption
