CARE COUNSELOR POSITION SUMMARY
In collaboration with the clinic care team, the Care Counselor supports implementation of the patient’s integrated clinical care plan, working towards resolution of patient’s clinically based needs / barriers. Additionally, the Care Counselor will conduct quality-focused health education / awareness and outreach to patients, educate existing and potential clients about health coverage programs, enroll clients into the appropriate coverage programs, assist with applications, and provide follow-up to ensure enrollment, utilization, and retention. In addition, the Care Counselor will make or record presentations to clients / patients on specific health-related topics to increase patient access to Social Determinants of Health (SDOH) assistance. Lastly, Care Counselors will function as members of the integrated care delivery team and will be responsible for communicating their progress towards resolving patient’s social determinants of health to avoid barriers to receiving quality care.

Provide strategic targeted outreach to explain Kheir’s Clinical Services as a Care Team member within the clinic.In collaboration with the clinic care team, the Care Counselor supports implementation of the patient’s integrated clinical care plan, working towards resolution of patient’s clinically based need / barriersPerforms population health management activities as assignedActs as liaison for the care teamPerforms Health Risk Assessment and initiates plans for eliminating social determinants of health identified in the clinically based assessmentsCoordinates continuity of patient care with internal and external healthcare organizations and facilities, coordinates information and care requirements with other care providersOutreach to educate individuals about health care options such as Medi-Cal, Family PACT, and other programs available to parents, teens, and the general public.Provide comprehensive assistance to children and their families, including in-reach and outreach to target members during the enrollment process for health insurance benefits and services that the organization provides.Follow up to verify enrollment and encourage the utilization of health insurance benefits and services.Connect with patients via text campaigns, and telephone outreach to assist with quality initiativesAct as a resource of information on the phone or in person, for reliable information regarding the clinic’s programs and conduct relevant case management.Participates in clinical care team meetingsSchedule appointments for patients (Primary Care, Dental & Optometry), and follow-up on any missed appointmentsFacilitate presentations on other health-related topics including, but not limited to, teen pregnancy prevention and STD’s, the importance of prenatal and pediatric care, preventative screenings, etc.Maintain records, logs, signing sheets, and other documentation required to track for contract compliance and evaluate program performance.Assist the supervisor with special tasks or events, including training, workshops, and material production.Ensures timely follow-up with provider post hospitalization / emergency room visit; retrieve discharge summaries and copies of medical recordsOther duties as assignedJob Type : Full-time
