Patient Care Coordinator
Full-Time, Non-exempt Position
ASSIGNMENT SUMMARY
The Patient Care Coordinator (PCC) will be responsible for developing and implementing organized care management procedures and coordinating inbound and outbound referrals at Kheir Center. He or she will be responsible for maintaining current knowledge of relevant compliance requirements, clinic operations, and managed care programs. He or she will collaborate with the care team and clinic leadership to ensure efficiency and accuracy in referral processing. S / he will assist the PCC team as needed with coordination for urgent referral follow-ups for patients' specialty care and will troubleshoot patient care issues and complaints. This person must have exceptional organizational skills with the ability to prioritize their daily workload and handle complex systems and information. The person must also demonstrate ethical conduct and maintain the confidentiality of patient information. The employee must perform job duties with sensitivity to the patient's diverse cultural backgrounds. The coordinator may also perform clerical functions as needed. He or she must be highly proficient in Microsoft Office programs and other computer functions in order to maintain detailed records. This is an hourly position.
MINIMUM QUALIFICATIONS
- Minimum High School diploma; Bachelor's Degree a plus but not required.
- 1 to 2 years of related experience in a fast-paced professional medical environment.
- Ability to exercise discretion to ensure that patient confidentiality is maintained and a professional, respectful clinic / department environment exists at all times.
- Familiarity with computer and Windows PC applications, which includes the ability to learn new and complex computer system applications.
- Excellent written and verbal communication skills.
- Excellent time management and organizational skills.
- Excellent computer skills, including email, Microsoft Office (Excel!) & data tracking.
- Ability to work quickly, follow directions and make independent decisions.
- Ability to maintain confidentiality when handling patient medical records.
- Experience in community clinic environment a plus.
- Bilingual English / Spanish or English / Korean required.
- Valid California driver license and insurable driving record required.
